 Measurement of prostate: volume/mass, ultrasound/radiation imaging

Well, the Subject line pretty much says it all.

How is the extent of an enlarged prostate measured, without previous
knowledge of the individual's unafflicted prostate size? Is a mass or
volume figure in use? Are ultrasonic or radiation methods in use? Are
other imaging methods in use?

My diagonsis is benign prostatic hyperplasia. My urologist has an
imager used for bladder volume measurements prior to flow meter tests.
It resolves tissues in a plane, a wedge shaped scan. The operator marks
the screen images from the two normal (perpendicular) scans and
computes bladder volume, before and after voiding.

Could that machine and that operator measure my prostate size? And how
often might insurance pay for the measurement?
