Only  the existing filling would have to be removed. If there is any
decay under the filling, then that would have to be removed also.
 An inlay is a great restoritive option.  Loss of tooth structure is
minimal, because no undercuts are needed to  " Lock" in the inlay.
The inlay is then bonded to your tooth .
