No.

No.

Would suggest you seek a referral to consult with a gastroenterologist.

It sounds like you are having paroxysmal nocturnal dyspnea.  This
symptom is associated with congestive heart failure.  For this reason,
would suggest you inform your doctor who will likely refer you to a
cardiologist.

Andrew,

That is indeed what I'm having.  Good diagnosis.

I can walk for two hours, no problem.  I can hike uphill reasonably
well as part of a two hour hike.  Does this imply my heart is in
decent shape?  Since I have had one real bad panic attack (the first
one) and maybe a couple minor ones over the past few months, I think
my symptoms are more likely due to this.  For example, in Tahoe when
I was offered an upstairs bedroom I rejected it out of hand when I
saw it was small, the walls tapered inwards (it was an A-frame) and
it was warm and stuffy.  All things that would drive my panic attacks
to new heights if I had had to stay in that room.

This is not to say the fact that I'm overweight 20-30 pounds does not
add to my problems.

I will mention this next time I see my physician.  Sooner rather than
later if it gets worse.
