 Need Info On Heart Valve Surgery Pt. (Ped, now forty, who is having BP

Hello!  One of my friends had heart valve surgery at age 5 to correct
a hole in her heart.  She is now forty, and the past 8 mos. have seen
some unusual problems.  Her {*filter*} pressure is WACKY.  She's tried
several meds, but nothing works.  One example, she takes her BP three
times a day and it can read:  193/180 and 90/48 all in the same day.
There seems to be no discernable pattern (waking or daytime or
evening) in the readings.  She's had bad asthma for years, so some of
the {*filter*} available to other patients are not available to her.  We
finally talked her into going to a specialist, not the GP she has been
seeing, and she's not too happy with the fact that the bills are
high, even though she has insurance, and many of the tests that they
are doing now are repeats of the tests that her GP did.  She did say
that they found that her bad cholesterol is high, and her good
cholesterol is low.  What would you
say is the prognosis for this type of patient?  I'm guessing that
it's pretty grim, especially if the BP defies control.  It sounds
like the heart valve is not operating properly, that the {*filter*} flow
inside the heart is creating a tamponade type of situation (because
of the narrow BP sometimes reported).  
How would you treat this patient, and what types of tests would
you do?  How soon would you do them?

........first have her {*filter*} pressure machine tested for accuracy.  It
sounds like the culprit and the false pressures could be a red
herring.  Coronary angiograms would tell how well her valve is
functioning , if her vessels are stenosed and if her hole repair is
still patent.  She is so young to have high cholesterol....and you
don't say what the measure is exactly so if it is fairly high she
would need meds to reduce it along with her dietary changes.
