Question on long term use of Amitryptiline

 Question on long term use of Amitryptiline

does any one know what the possible effects of long term use of
Amitryptiline are?  By long term I mean in the five to ten year range, with
a dosage of 75mg per day for the first five years tapering to 10mg over the nect
five years.

Dear Barry,

Long term use of amitryptyline carries no specific problems to mind

Its not {*filter*}ive, there is no tolerance to the antidepressant
effects.

The dry mouth/dry eyes/constipation/dizziness on standing type
side-effects do disappear to a greater or lesser extent within
a few days to weeks, but this is not associated with a loss in
potency of the drug.

Any residual side effect, of course, may not actually decrease
over months to years, but things shouldn't get worse.

I've never seen any long term side effects from amitryptiline.
I think it is very safe.

Tricyclics as a rule are safe, but may cause problems in
elderly patients and patients with cardiac problems. In general,
I think most clinicians would prefer nortryptiline to amitryptiline
because of these considerations. I am not aware of any cost
difference.

Doxepin has relatively low cardiotoxicity and is a useful alternative
to amitriptyline as it retains the sedative effect.  Nortriptyline
produces relatively little sedation and is more suited to withdrawn
patients whereas the more sedative tricyclics are more suited to
agitated patients.
