State disciplines health care providers
OLYMPIA ? The state Department of Health has taken the following disciplinary actions
against health care providers. The department regulates the practice of health professions
and enforces health and safety laws to protect the public from negligent, incompetent, or
illegal health care practices. Consumers who think a health care provider acted
unprofessionally are encouraged to file a report.
By calling the Health Professions Quality Assurance Division at the state Department of
Health, people can get the disciplinary history, license status, and number of complaints
against any credentialed health care professional in the state.
To place a complaint against a health care provider or get disciplinary information about
a doctor, dentist, or other provider, call the Health Professions Quality Assurance
Division at (360) 236-4701.

In December 2001, the Veterinary Board withdrew charges against Scot A. Lubbers, a
veterinarian in Clark County (VT3321).
Okanogan County
In December 2001, the Osteopathic Medicine and Surgery Board withdrew charges against
Edward L. Van Tassel, an osteopathic physician in Okanogan County (OP1339).
Spokane County
In September 2001, the {*filter*} Family Home Provider Program charged Nemesia S. Armstrong, an
{*filter*} family home provider in Spokane County (FP1322), with unprofessional conduct.
In February 2001, the Department of Social and Health Services revoked the licenses of the
facilities located at 12122 East 12th Avenue and 11601 East 12th Avenue in Spokane.
In October 2001, the Nursing Assistant Program denied the application for certification
and revoked the registration of Lameatria Clark, a nursing assistant in Spokane County
(NA111163), with no right to reapply for three years. She was charged with unprofessional
conduct for misappropriating a money order, which belonged to a patient.
In October 2001, the Nursing Assistant Program revoked the license of William E. Dellwo, a
nursing assistant in Spokane County (NC10055538), with no right to apply for 10 years. He
was charged with {*filter*} abuse of a child.
In August 2001, the Nursing Commission charged Cecilia A.{*filter*}son, a registered nurse in
Spokane County (RN117225), with incompetence, negligence or malpractice, violating
standards of nursing conduct or practice, and diverting narcotics for her own use.
In August 2001, the Nursing Commission indefinitely suspended the license of Nora E.
Estes, a practical nurse in Spokane County (LP43657). She was charged with unprofessional
conduct for failing to comply with an order by the disciplinary authority.
In September 2001, the Pharmacy Board revoked the license of Meegan Evans, a pharmacy
technician in Spokane County (VA41864), with no right to reapply for five years. She was
charged with diverting controlled substances for her own use.
In August 2001, the Nursing Commission placed Clifford Grindley, a registered nurse in
Spokane County (RN85981), on probation for 36 months. The terms of his probation include
paying a fine, submitting personal progress reports, and completing courses in
supervision, delegation and basic assessment. He was charged with failing to supervise
staff, assure adequate staffing, and failing to administer appropriate patient care.
In September 2001, the Unlicensed Practice Program issued a cease-and-desist order against
Jonathan Hall, a Spokane County resident engaging in unlicensed practice as a registered
counselor. He owns and operates a counseling service using the name Sacred Psychology.
In August 2001, the Nursing Assistant Program denied the certification application and
revoked the registration of Janet M. Harding, a nursing assistant in Spokane County
(NA00109310), with no right to reapply for at least five years. She was charged with
unprofessional conduct for failing to disclose criminal convictions on her application,
possession of a controlled substance, and use of a building for an unlawful drug purpose.
In October 2001, the Nursing Assistant Program suspended the certification of Joyce M.
Hay, a nursing assistant in Spokane County (NC10000381). The suspension is stayed provided
that she complies with certain terms including submitting quarterly performance reports,
and the payment of a fine. She was charged with negligence.
In August 2001, the Pharmacy Board approved the application of Stacie Hoover (IR43175) to
practice as a pharmacy intern in Spokane County and placed her on probation because of a
past criminal conviction. The terms of her probation include attending counseling sessions
and submitting quarterly reports from a preceptor.
In October 2001, the Nursing Assistant Program withdrew charges against Gregory Hughes, a
nursing assistant in Spokane County (NA00115654). In February 2001, he was charged with
unprofessional conduct for abuse and for failing to reveal a conviction for delivering
amphetamines on his registration application.
In August 2001, the Nursing Assistant Program charged Paula Irene Johnston, a nursing
assistant in Spokane County (NC10055898), with unprofessional conduct for abusing a
nursing home resident. She is permanently placed on the OBRA Nurse Assistant Registry and
nursing homes may not employ her.
In August 2001, the {*filter*} Family Home Provider Program charged William D. Johnson, an
{*filter*} family home provider in Spokane County (FP5579), with unprofessional conduct. In
January 2001, the Department of Social and Health Services revoked the license of the
facility located at East 2822-35th Avenue in Spokane.
In August 2001, the Nursing Commission indefinitely suspended the license of Sharon L.
Lally, a nurse in Spokane County (LP36497). In March 2001, she was charged with
unprofessional conduct for using a false registered nursing license to obtain a job and
assuming the job responsibilities of a registered nurse without a valid license.
In July 2001, the Nursing Assistant Program revoked the certification of Chalise P.
Lobdell, a nursing assistant in Spokane County (NC10057070), with no right to reapply for
10 years. She was charged with unprofessional conduct for abusing a nursing home resident.
In September 2001, the Counselor Programs charged Alonda L. Moran, a counselor in Spokane
County, license number LH6771 and registration number RC12699, with alleged dishonesty,
corruption, incompetence, negligence or malpractice, and abuse of a client or patient.
In October 2001, the Nursing Commission charged John G. Parr, a nurse in Spokane County
(RN107014), with dishonesty or corruption, incompetence, negligence or malpractice,
misrepresentation or fraud, and violating standards of nursing conduct or practice. It is
alleged that he performed negligent treatment and failed to follow accepted standards of
practice.
In September 2001, the Nursing Assistant Program granted Angel M. Payne the registration
to practice as a nursing assistant in Spokane County (RA00020018), but placed her on
probation for one year. She was charged with unprofessional conduct due to a criminal
conviction for possession of drug paraphernalia.
In September 2001, the Nursing Assistant Program denied the registration application of
Patrick J. Pedersen (RA00019858), a nursing assistant applicant in Spokane County, with no
right to reapply for three years. He was charged with unprofessional conduct for failing
to reveal convictions for {*filter*} and reckless endangerment on his application.
In August 2001, the Medical Commission served amended charges on Ronald D. Springel, a
physician in Spokane County (MD24392), for unprofessional conduct due to impairment.
In October 2001, the Dental Commission charged Patrick J. Stevens, a dentist in Spokane
County (DE5806), with unprofessional conduct for allegedly self-prescribing controlled
substances and/or legend {*filter*}, practicing with an expired license and failing to
cooperate with the disciplining authority.
In August 2001, the {*filter*} Family Home Provider Program charged Eileen K. Thomas, an {*filter*}
family home provider in Spokane County (FP405), with unprofessional conduct. In November
2001, the Department of Social and Health Services revoked license of the facility located
at 2217 East Pacific, Spokane.
In June 2001, the {*filter*} Family Home Provider Program restricted the registration of Marie
T. Thompson, an {*filter*} family home provider in Spokane County (FP5546) She may not operate
an {*filter*} family home located at 110 E. Ballard Road in Colbert, unless the Department of
Social and Health Service reinstates the facilitys license.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to
respond to Department of Health in writing. The case then enters the settlement process.
If no disciplinary agreement can be reached, the case will go to a hearing.
