Hello everyone,

I'm about to go on holiday to Vietnam, and my doctor advised me to take
the malaria-precaution medicine called LARIAM. No i've heard some good
things about this medicine and some bad things. I'm hoping that some of
you here can answer a few questions for me.

1: does anyone know of the more harmful side-effects of Lariam, such as
developing paranoia, or other malfunctions of the mind?

2: does anyone know for sure how often the more harmful side-effects
occur? The Laroche Company claim it's only 1 in 10.000 cases, yet some
docter in Holland said it's more like 1 in every 2.500 cases. The less
harmful side-effects occur at a rate of 60%, he claims!

3: does anyone know of people who suffered some real serious problem
from taking this medicine?

I hope someone can help me with this, because I'm not sure if I should
take this as a precaution, the side-effects scare me sriously!

Dear Mr Kempen,

1) Lariam ( mefloquine) is the drug of first choice to prevent malaria infection in
Vietnam. The dangerous area's are: The Red River Delta, the coast north of NHA TRANG
en the southern area's.

2) The side effects of Lariam are the useal: headache, nausea, bradycardia ( slow
heart rate) insomnia ( sleeping disorders) . All these side-effects are minor,
disappear and are no reason to stop the drug.
The serious side-effects are: Hallucinations, paranoia , co?rdination disturbances.
However these side-effects are very rare  and occur mostly during THERAPEUTIC
USE.(higher doses).
In PROFYLACTIC USE (like you need) these side effects are rare ( this probably
explaines the different figures you mentioned).
There are a few diseases wich contra-indicate the use of LARIAM ( cardiac rythmic
disorders,serious liver function disorders, epileptic disease, recently use of
halofantrine and kinine, ask your doctor) and interactions with {*filter*}antidiabetic
{*filter*} and anticoagulantia.
All the problems you can avoid by starting LARIAM  THREE WEEKS BEFORE TAKE OFF to
Vietnam. ( 250 mg once a week after meal). If you notice any problems during these
period than stop the drug. The side effects are reversible and there is another
option for prevention of malaria.
You have to take the medication until four weeks after leaving the malaria area's .

3) Have a good trip and mind you: traffic accidents are the big killer

ALthough I cannot gurantee 100% for *everyone*, in the case of my
older kid (the younger one was given someother similar drug) we had her
on LARIAM last year and this year when we went on an extended vacation to
India, Honk Kong, singapore etc.  SHe suffered no side effects and was a
happy camper. My little one (last year was 5 months, and this year 1.7)
was given some "??quin" (I think it was meloquin) had some diahrrea for just
one day when she had to take that.  Since these are taken only once a week
(2 weeks prior, during and 4 weeks after return), it wasn't too bad.
Check with your local Travel clinic.  They will have lot of good info.
on other things too, like Hepatitis A vaccine etc.
