One can imagine this Arab guy, who isn't infective until he has red bloches all over him and can hardly walk, trying to make it through customs. "Mosquito bites. Gurgggle. Horrrible fishing trrrip, by Allah!" Hmmm. Alternately, one can imagine this guy making it though customs before his symptoms appear, and then staggering around shopping malls in the few hours he can still walk, with these oozing pustules on every square inch, trying to touch people and cough on them. Why, he might expose dozens of people before getting picked up and diagnosed! Who will then get vaccinated, and that will be that. To achieve widespread infection an aerosole delivery would have to be used. Spraying it widely over an entire city via crop duster wouldn't work since it would disperse too widely in the open air, but a large-scale attack with thousands of victims could be mounted by dispersing it in buildings and subways in a major city. I wouldn't be quite so sanguine about smallpox as the others have. The sudden appearance of, say, 5000 smallpox patients would overwhelm any city's medical resources and produce 10's of thousands of secondary cases (many of whom would have left the area before the outbreak was detected). Given that the US public health service has been greatly reduced in the last few decades, controlling a major outbreak wouldn't be as easy as it was made to sound. And if just a few of these infected persons carried the disease to the third world, the results could be disasterous.

Within the last two years, I obtained the pneumovax vaccination. I have never suffered any significant untoward reaction from any of these vaccinations and am in very good health. One should always plan for the worst and most unexpected type of attack from an enemy so as to have a "plan of action" in advance to deal with any kind of attack, including a biological attack.
