 Gastric reflux disorder-anyone have surgery?

Has anyone had surgery for a severe case of GERD?  I have been on
Prilosec for a year and it works wonders, but as soon as I stop it is
bad news.  My doctor now wants me off from it permanently at this
point because of the possible cancer side effects and my age (41) and
is suggesting surgery.  Has anyone out there gone thru the surgical
procedure?  What problems are you having (if any) since surgery?  Any
suggestions would be appreciated.  Thanks, Terry at

The standard operation for persistent reflux esophagitis unresponsive
to management with medicines is a "wrap" of the upper part of the
stomach upon itself (Nissen Fundoplication). It is highly successful
in recreating the physiology of the lower esophageal sphincter.

I assume you've had esophagoscopy demonstrating lower esophageal
ulcers, or a 24 ambulatory ph test to confirm acid reflux, and
esophageal manometry to rule out other pathologic conditions.

As to the operation, LOOK INTO THIS CAREFULLY!! The standard operative
approach has been via a big midline incision. 4-6 days in the
hospital, 6 week recovery. There are a number of surgeons that are
doing this laparoscopically. Exactly the same operation, but done
through four 1/2 inch punctures. The major downside is that operation
takes longer, (2 1/2 hours instead of 1 hour). On the other hand,
hopsital stay is about 2-3 days, and return to prehospital activity in
10-14 days. The reduction in pain with this method can be dramatic
compared to the standard open procedure.

One of your key questions for the surgeon is whether or not you're a
candidate for this operation. He may say no because he doesn't know
how to do it, so make sure you're prepared to get a 2nd opinion. Look
for a surgeon VERY experienced in laparoscopy.

Good luck,
H McCollister MD

I've not had the surgery but have participated in a number of
procedures as the first assistant...

The operation is called a Nissen Fundoplication...

Basically, the stomach is wrapped over onto itself, tightening the
lower esophageal spincter (where the cooking.net">food tube meets the stomach).

It is s nice operation in the right hands.  It can be done via the
laparoscope...meaning 4 small incisions ( 1-2CM length) in your belly
rather than a zyphoid to sub-umbilical incision ...

I would want to know from the surgeon how many he has done, where he
got his training, who will assist him, his "success rate"..ie:how many
patients are symptom free after, say, one year post-operative.

We have a couple very good surgeons for that procedure here in
Rochester, NY...there are probably a couple in your vacinity also..

Good luck!
