A 59-year old man, heavy smoker, was brought to hospital at 4am with a 3hr
history of crushing, band-like chest pain.  He was seen immediately but died
while being examined.
Past medical history:-
-diet controlled DM(type 2)
-hypertension
-intermittent claudication
-several attacks of chest pain over the 5yrs before death

what is the likely cause of death?
why was no evidence of myocardial infarction found during post-mortem?

Is it just me, or do others think this looks a lot like somebody's
homework question?

Hmmmm.... cardiac arrest?

Perhaps because there was no myocardial infarction?
